CareAxis Sr. provides elder care services targeted to the specific needs of each individual or family. The focus is on assisting the elder person through an experience or event that is impacting their life or threatening their independence. Often this is a medical event, which creates obstacles to continuing their lifestyle, as they knew it. CareAxis Sr. works with the elder person, the physicians and medical caregivers, ancillary services and family members to assist in developing a care plan based on individual needs.

	Formulation of Care Plan
	Coordination of home services
	Attend physician appointments for clarification of needs, medication dispensing, updating plan of treatment, etc.
	Coordination/Implementation of medical treatment as prescribed by physician
	Secure appropriate in-home care and medical equipment as prescribed by the physician
	Weekly contact with the client to evaluate the quality of services being rendered and assess needs
	Additional services appropriate to individualized care plans
